An exciting opportunity has arisen for an experienced clinical practitioner to join Saint Francis Hospice in a newly revised role to support the Single Point of Access (SPA) & Referrals Hub Service –’the front door’ for all referrals to the hospice – in ensuring skilled, efficient and effective triaging. Saint Francis Hospice is a dynamic, forward‑thinking organization delivering a full range of specialist palliative care services for all individuals with life‑limiting illness. We achieved ‘Outstanding’ in our CQC inspection in August 2024 maintaining what we achieved in 2016. We have recently adopted an exciting 5‑year strategy to reconfigure our services to meet the changing needs of our local community and ensure we are reaching all individuals that require palliative care expertise.
Responsibilities
- Responsible for individual holistic clinical assessment of referred individuals.
- Promote clinical leadership and demonstrate a high standard of skills.
- Ensure safe and effective practice over a flexible 7‑day rota.
- Enhance the service user experience and contribute to effective leadership and collaborative team working.
- Contribute to the delivery of the organisation’s objectives and new strategy.
- Work closely with the SPA & Referrals Hub Manager, Referrals Hub Clinical Nurse Specialist, Referrals Assistant and Referrals & Admissions Coordinator.
- Work pro‑actively across all service areas to ensure plans and arrangements following assessment are in place for acceptance and admission to appropriate hospice services.

- Specialist clinical experience in palliative care.
- Team player with strong interpersonal and communication skills.

Job Details
- Salary: £45,502 to £52,884 per annum, depending on experience
- Hours: 37.5 per week, full time
- Location: Main hospice site in Havering‑atte‑Bower, RM4 1QH
- Contract type: Permanent
