Surrey and Borders Partnership NHS Foundation Trust is seeking a Discharge Practitioner to coordinate ward-based discharge activity across allocated wards at Farnham Road Hospital and Silverwood, working with MDTs and bed flow teams to reduce delays.
The postholder will engage with Local Authorities, HTT, CMHRS, Housing Support Leads and contracted beds to progress discharge plans from admission to discharge, attend ward rounds and MDT meetings, and maintain SystmOne records.
