The predicted salary is between 40000 - 48000 £ per year.

This role is offered on a permanent basis. The hours are part-time, 34 hours per week. Work with individuals who are rough sleeping or residing in temporary accommodation to understand their personal and often very sensitive difficulties. Act as a clinical leader in the delivery of a service for those experiencing both homelessness and complex health needs to patients, ensuring that the needs of the patient are the priority. Lead in the planning, development and delivery of a service for those experiencing homelessness and complex health needs. Provide line management and supervision to staff.

Responsibilities

  • Manage the referral pathway for Multiple Complex Needs including Mental Health.
  • Chair partnership team for Multiple Complex Needs including direct partners Adult Social Care (ASC) and VIA substance misuse service.
  • Complete and facilitate Mental Health assessments, risk assessments and complex needs assessment with individuals referred to service.
  • Work with Housing, Adult Social Care, VIA and other relevant partners to consider complex needs, risk assessments, access requirements and referrals required to mainstream services.
  • Work with senior management, commissioners and partners to implement NICE guidelines NG214 Integrated Health and Social Care for people experiencing homelessness within the Trust, the team and wider services.
  • Develop a method and effective measurement of complexity to reflect needs of the cohort.
  • Work in partnership with Commissioners and Partner agency senior management to monitor and develop the service.
  • Complete KPI returns.
  • Develop methods of coproduction with experts by experience.
  • Consider issues of inequality, disability access, capacity, disadvantages met by this cohort and look at creative solutions.
  • Challenge discriminatory practices and barriers to services for this disadvantaged cohort.
  • Support partners in considering specific needs and support requirements that will contribute to obtaining and maintaining housing.
  • Supervise GHC, GCC & VIA staff members including Nursing, Recovery Workers, Social Care Practitioners, OT and Admin.
  • Facilitate supervision of Psychology staff via Psychology team support structures.
  • Profile current cohort in terms of needs and contribute to data collection regarding Mental Health, Complex needs and access requirements.
  • Provide information, advice and guidance regarding Housing & Homelessness to Trust colleagues and support the development of appropriate training.
  • Develop links with Housing colleagues including District Councils, General needs Supported Accommodation providers, Housing Associations and provide support and advice regarding Mental Health services.

Qualifications & Experience

  • Essential: Relevant professional Nursing / Social Work / Occupational Therapy qualification.
  • Desirable: Management qualification or relevant experience.
  • Essential: Experience of working with people with a mental health condition in a variety of settings including the community.
  • Essential: Experience of working with people with a mental health disability and mental health problems who have behaviours which challenge.
  • Desirable: Experience of working in the housing sector and with housing partners.
