Dementia Matron – Complex Care at Home Team

Join to apply for the Dementia Matron – Complex Care at Home Team role at Saarthy Travel Solutions.

Role is offered on a fixed term/secondment basis for 6 months. The hours are part‑time, 15 hours per week.

Responsibilities

  • Provide autonomous practitioner care for allocated patients, including triage, planning, management and coordination of complex needs within patients\’ homes.
  • Lead clinical care planning and coordination for individuals with highly complex dementia, long‑term conditions and frailty.
  • Collaborate with primary and secondary care services, statutory and non‑statutory agencies to develop personalised care plans and advocate the \”What matters to you\” principle.
  • Apply a case‑management approach to prevent avoidable deterioration, reduce unplanned hospital admissions and support self‑management for patients and carers.
  • Gather and interpret complex clinical data to conduct comprehensive health and social care needs assessments, facilitating multi‑agency partnership case management.
  • Provide specialist knowledge, support and guidance to multidisciplinary team members on mental health, depression, anxiety and other complex mental health issues.
  • Use health coaching and motivational interviewing skills to enable appropriate self‑management and problem‑solving for patients and carers.
  • Ensure patients with cognitive impairment receive equitable access to GHC specialist memory assessment services and adhere to the Complex Care at Home Dementia Pathway (2019).
  • Act as an autonomous mental health specialist, linking with primary, secondary and territory services to promote equitable treatment and appropriate care pathways.
