An exciting opportunity has arisen on Woodland View Ward in the Forest of Dean community hospital for a band 5 registered nurse. The role is on a permanent basis, and the hours are 37.5 per week. We welcome applications from newly qualified individuals.

Woodland View Ward is a 24 bedded inpatient ward that is responsible for the care of patients requiring rehabilitation, sub-acute and end of life care. The medical model is a mixture of Doctor and Advanced practitioners; we also have a multidisciplinary team based on the ward.

You will be responsible for delivering safe and effective nursing care to our service users. You will be expected to take responsibility for your own professional development and will be supported by the Trust to achieve development opportunities as appropriate.

Alongside a variety of responsibilities, you will be assessing, planning, implementing, and evaluating care for a specific group of patients. Undertaking comprehensive health care needs assessment of patients and reassigning as appropriate. You will undertake joint assessments and be responsible for initiating, co-ordinating and implementing discharge planning involving the Multi-disciplinary, Primary care, Mental Health and Social care Teams where indicated, making onward appropriate referrals.

Responsibilities

  • Ensure all clinical care requirements of patients are communicated effectively at ward handover to allow seamless provision of care from shift to shift.
  • Responsible for ensuring patients records are kept up to date, accurate and in line with the Trust's Record Keeping Policy and Medicines Management Policy.
  • Responsible for assessing, planning, implementing and evaluating care for a specific group of patients. Undertaking comprehensive health care needs assessment of patients and reassigning as appropriate.
  • Undertake joint assessments and responsibility for initiating, co-ordinating and implementing discharge planning involving the Multi-disciplinary, Primary care, Mental Health and Social care Teams where indicated; making onward appropriate referrals.

Preceptorship and Training

We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module Preceptorship for Healthcare (AP7026). Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.
