We are looking for a compassionate and skilled Community Registered Nurse to join a dedicated Community Nursing Team, delivering high-quality palliative and end-of-life care within patients’ own homes and care settings. This is a highly rewarding, hands-on role where you will provide holistic, person-centred care to patients and their families, while also offering telephone advice and support via a single point of access service. You will play a key role in supporting both patients and healthcare professionals, acting as a clinical resource to promote best practice in palliative care across the community.

Key Responsibilities

  • Deliver holistic palliative nursing assessments and hands-on care in community settings
  • Plan, implement, and evaluate person-centred care plans in partnership with patients and families
  • Provide responsive and timely care, including urgent interventions when required
  • Offer emotional, psychosocial, and spiritual support to patients and families
  • Provide guidance and reassurance via telephone support services
  • Promote patient choice, including preferred place of care and end-of-life planning
  • Act as a clinical resource and educator to health and social care professionals
  • Work collaboratively within a multidisciplinary team
  • Maintain accurate documentation and contribute to audits and service improvement
  • Adapt to the needs of the service with a flexible and proactive approach

Candidate Requirements

  • Registered Nurse (NMC – Part 1 or 2)
  • Recent experience within palliative care, oncology, or end-of-life care
  • Proven ability to support patients and families through complex and sensitive situations
  • Experience working within a community setting (desirable)
  • Excellent communication skills, both face-to-face and over the phone
  • Strong organisational, IT, and problem-solving skills
  • Full UK driving licence
