Advanced Practice Nurse (Regional Float, South) FT Primary Care Service Line Oceanport Req #: 0000171384 Category: Physician Assistant Status: Full-Time Shift: Day Facility: Barnabas Health Medical Group Department: Primary Care APN Float Pool Location: Barnabas Health Medical Group, 2 Crescent Pl C, Oceanport, NJ 07757 Job Overview: RWJBarnabas Health, Primary Care Service Line is seeking Full-Time Advanced Practice Providers, Nurse Practitioners or Physician Assistants, to provide regional coverage within the primary care service line. Qualifications: Current NJ APN/PA license and certification required. Master\’s degree or Graduate Level Certification required. Certified Nurse Practitioner CDS, DEA, and BLS required. Proficient in database and reporting tools such as Microsoft Word, Excel, and PowerPoint. Scheduling Requirements: Monday-Friday, Day shift Regional Onsite Coverage Essential Functions: Perform comprehensive initial and episodic health assessments to identify health problems. Develop and implement individualized nursing and medical care plans in collaboration with the interdisciplinary team. Initiate diagnostic, therapeutic, medical, and nursing actions with physicians. Provide follow-up care, assess progress, and recognize emergent issues. Monitor and adjust medications per protocol, and implement nutritional therapies as needed. Triages patient calls requiring medical attention. Order lab and diagnostic procedures; interpret findings. Document all patient information accurately in charts. Provide health education to patients and families. Manage medical emergencies according to standing orders. Ensure continuity of care across healthcare settings. Offer counseling and referrals for psychological or social issues. Serve as a resource and educator for healthcare team members. Collaborate with partner sites to coordinate care. Evaluate care quality and patient outcomes, and suggest interventions. Travel as needed for onsite and telemedicine support within the regional territory, with mileage reimbursement.
