We are looking for an Integrated Support Worker to join our Community Care Service, supporting the coordination of care and support for people receiving services from Micare, with a particular focus on those receiving support from multiple Health and Social care services, such as those at the end of their life.
About the role…
- Acting as a single point of contact for the person receiving services, removing duplication and ensuring the person only tells their story once.
- Liaising with a multi-disciplinary team across Health and Social care services to bring together a range of support to one place.
- Support the coordinators with implementing care and support across all pathways to include Crisis Response, Reablement, D2A, C2A, safety net and complex care. This will also include first visits assessments, which means you will be required to be available during your working hours to go out into the community at short notice.
- Cover calls for Micare if there is a need and attend meetings both face to face and virtually when required.
About you…
- An experienced social care worker, you will be passionate about care at the end of life, empathetic, caring and responsive to people’s needs, wishes and preferences.
- NVQ 3/QCF in Social Care or willingness and ability to achieve within agreed timescale.
- Flexibility in working patterns in order to ensure the needs of the people receiving support are met. Your working hours will include weekend working every other week.
- You will be able to work from various locations across Rutland to include People’s homes, Rutland Memorial Hospital, Uppingham Surgery, Catmose House and Oakham Enterprise Park. Additional locations may be included as per service development.
The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.
