We are looking for an Integrated Care Coordinator to join our RISE Team at Rutland County Council. The post holder will work as part of a dynamic multi‑agency integrated team. The service supports anyone living with long‑term health conditions, whether receiving social care funding or privately funding their own care. Our RISE Integrated Care Coordinators give guidance on frailty and fall prevention, aids and equipment, healthy living, and local support services.

The role of an Integrated Care Coordinator in RISE is varied; you will be supporting adults aged 18+ living within the Rutland community, adopting a creative and empathetic approach. The post holder must have the ability to operate within integrated health and social care systems and third‑sector providers. About you

  • Level 1: Educated to A’ Level/NVQ3 standard or equivalent or Level 2 and 3: Educated to NVQ4 standard or professional qualification (e.g. social work, nurse, OT, physio), educated to degree level.
  • Work in a Strength Based Approach and Outcome focused way.
  • Thorough understanding of Community Care Legislation and understanding of multidisciplinary working.
  • Knowledge and experience of safeguarding policy and procedures.
  • Experience in an adult social work/adult health environment.

Responsibilities

  • Proactively work with health and social care colleagues to identify people with multiple long‑term conditions at risk of hospital admission.
  • Work within a multidisciplinary, integrated team to support Rutland residents or those registered with a Rutland GP to co‑produce simple, personalised care and support plans, linking them to community and statutory services.
  • Complete combined health and social care assessments at home where appropriate, providing information and support around care options and helping individuals manage and improve their health and wellbeing.
  • Coordinate services in partnership with health professionals to ensure individuals receive the right support for their needs.
  • Support people registered with a Rutland GP to reduce Adult Social Care and healthcare contacts by encouraging engagement with appropriate services to optimise health outcomes.
  • Empower people to take control of their health and wellbeing using a holistic, person‑centred approach, connecting them to community and statutory support and helping sustain or develop local community groups.

The successful applicant for this post will be subject to an enhanced check by the Disclosure and Barring Service (DBS). The costs for the DBS are paid by us.
