Job Summary

We are seeking to recruit an experienced and motivated Practice Nurse to join our team at Runnymede Medical Practice working 20-30 hours per week.

Main duties of the job

As a highly valued member of the multidisciplinary workforce your duties would involve the full range of Practice Nursing duties with an emphasis on long term condition care, cervical screening and childhood immunisations.

• Support in the long‑term condition management within the Practice, including being involved with quality improvement and assurance initiatives
• Teach and advise patients and their families on how to manage their condition
• Lead on public health campaigns e.g. flu programme
• Contribute to Practice targets (QOF etc), complying with relevant guidance
• Support in the delivery of enhanced serviced and other service requirements

Other Responsibilities

• Support the Infection Prevention Control (IPC) lead and work to the IPC Policy
• Develop, implement and embed an effective care management programme for the frail and house‑bound patients registered with the organisation
• Embed population health management approaches within general practice, targeting health inequalities and improving access
• Support new‑to‑practice nurses and other healthcare professionals within the Practice
• Chaperone patients where necessary
• Support the practice team with the reviewing and implementation of practice policies and protocols, ensuring conformance to extant legislation
• Contribute to and embrace the spectrum of clinical governance
• Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children
• Recognise the boundaries of their practice and know when and to whom patients should be referred
• Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through incidents and near‑miss events
• Work within national and local protocols
• Participate in clinical audits and research projects and implements changes as required, including supporting with the development, and updating of practice protocols/guidelines and procedures locally
• Support public health and screening programmes, including immunisations, vaccinations, and cervical screening and also provide opportunistic health promotion advice

Clinical Skills & Experience – Essential

• Cervical cytology

Clinical Skills & Experience – Desirable

• Chronic disease management

Qualifications – Essential

• Registered Nurse with current NMC registration
• Cervical Screening qualification

Qualifications – Desirable

• General Practice Nursing Certificate

Experience – Essential

• Experience working within General Practice
• Experience of chronic disease management
• experience of infection prevention and control measures

Experience – Desirable

• Understanding of the current NHS landscape, specifically primary care

Knowledge & Skills – Essential

• Understanding the importance of evidence-based practice
• Ability to promote best practice regarding nursing matters
• Ability to work within own scope of practice
• Understanding of safeguarding of adults and children
• Strong organisational and time management skills
• Excellent communication
