About the role
The Housing Advice and Enabling Team provides support to those who are at risk of homelessness, are homeless or need help to pay their rent and council tax.
As an Outreach Worker you’ll be supporting residents who are currently in hospital and have housing issues that prevent their discharge through using your understanding of the services provided within Social Care to the discharge of hospital in-patients.
You will be providing help and advice regarding homelessness and housing at external locations around the Borough.
About you
In addition to previous experience of providing accommodation-based support and wellbeing support to clients, you will need to demonstrate an ability to problem solve and work with partner agencies to put clients truly at the centre of what we do in line with our CANDO values.
