Our client in Central London is seeking a dedicated Locum Social Worker to join the Hospital Team. Key Responsibilities:
- Undertake assessments under the Care Act 2014 to facilitate timely hospital discharges.
- Work closely with health professionals, families, and community services to develop effective discharge plans.

- Ensure safeguarding concerns are identified and managed in line with legislation and local procedures.
- Maintain accurate case records and documentation in line with council and professional standards.
Requirements:
- Qualified Social Worker (Degree/DipSW/CQSW)
- Current registration with Social Work England
- Previous experience in hospital discharge or adult social care services
- Strong knowledge of the Care Act 2014, Mental Capacity Act, and safeguarding protocols
- Ability to work under pressure in a fast-paced, multi-disciplinary environment
