Are you a qualified Social Worker with experience supporting older people and adults with physical disabilities ?
We are currently looking for a dedicated and compassionate Social Worker to join a supportive team, helping adults to maintain their independence, dignity and quality of life.
The Role
You will work with older people and adults with physical disabilities, completing assessments, coordinating care and support, and helping individuals achieve the best possible outcomes.
You will be responsible for:
Completing Care Act assessments and reviews.
Assessing individuals' care and support needs.
Developing person-centred support and care plans.
Supporting adults to remain as independent as possible within their communities.
Working with individuals, families and carers to identify appropriate support.
Coordinating packages of care and liaising with care providers.
Supporting hospital discharge and transitions where required.
Working alongside health professionals and other agencies as part of a multidisciplinary team.
Identifying and responding appropriately to safeguarding concerns.
Managing a varied caseload and maintaining accurate case records.
Ensuring all work is completed in accordance with relevant legislation, policies and procedures.
About You
We are looking for a Social Worker who:
Holds a recognised Social Work qualification .
Is registered with Social Work England .
Has experience working with older people and/or adults with physical disabilities.
Has a good understanding of the Care Act 2014 , safeguarding and person-centred practice.
Has experience completing assessments, reviews and care planning.
Is confident working with individuals, families, carers and professionals.
Has excellent communication and organisational skills.
Can manage a caseload effectively and work independently.
Is passionate about promoting independence, choice and wellbeing.
