Are you a qualified Social Worker with experience supporting adults with mental health needs?
We are currently looking for a compassionate and experienced Mental Health Social Worker to join a dedicated team and support individuals to achieve greater independence, stability and wellbeing.
The Role
As a Mental Health Social Worker, you will work with adults experiencing a range of mental health needs, providing person-centred support and helping individuals access the appropriate services and care.
Your responsibilities will include:
- Completing assessments and reviews of individuals' care and support needs.
- Developing person-centred care and support plans.
- Supporting individuals to achieve positive outcomes and maintain their independence.
- Working with individuals, families and carers to understand their needs and identify appropriate support.
- Managing a varied and complex caseload.
- Working closely with mental health professionals and multidisciplinary teams.
- Liaising with health services, housing providers, voluntary organisations and other agencies.
- Identifying and responding appropriately to safeguarding concerns.
- Supporting individuals through transitions and changes in their care.
- Maintaining accurate and up-to-date records and case notes.
- Working in accordance with relevant legislation, policies and safeguarding procedures.
About You
We are looking for a Social Worker who:
- Holds a recognised Social Work qualification.
- Is registered with Social Work England.
- Has experience working with adults experiencing mental health difficulties.
- Has experience completing assessments, reviews and care planning.
- Has a good understanding of safeguarding and relevant social care legislation.
- Is confident managing a complex caseload.
- Has excellent communication and interpersonal skills.
- Can work effectively as part of a multidisciplinary team.
- Is compassionate, resilient and committed to supporting people to achieve positive outcomes.
