Are you a qualified Social Worker with experience supporting adults with learning disabilities?

We are currently looking for a dedicated and compassionate Learning Disabilities Social Worker to join a supportive team and help individuals achieve greater independence, choice and control over their lives.

The Role

As a Learning Disabilities Social Worker, you will work with adults with learning disabilities, their families and carers to assess needs, coordinate appropriate support and promote positive outcomes.

Your responsibilities will include:

  • Completing Care Act assessments and reviews.
  • Developing person-centred care and support plans.
  • Supporting individuals to maximise their independence and achieve their personal goals.
  • Working closely with individuals, families and carers to understand their needs.
  • Coordinating care packages and liaising with care providers and other services.
  • Supporting individuals with transitions, including moving between services and accommodation.
  • Working alongside health professionals and multidisciplinary teams.
  • Identifying and responding appropriately to safeguarding concerns.
  • Supporting individuals with complex needs where required.
  • Promoting choice, control, dignity and independence.
  • Maintaining accurate and up-to-date case records.
  • Ensuring all work is carried out in line with relevant legislation, policies and procedures.

About You

We are looking for a Social Worker who:

  • Holds a recognised Social Work qualification.
  • Is registered with Social Work England.
  • Has experience working with adults with learning disabilities.
  • Has experience completing assessments, reviews and support planning.
  • Has a good understanding of the Care Act 2014 and safeguarding.
  • Understands person-centred approaches and promoting independence.
  • Is confident working with families, carers and multidisciplinary professionals.
  • Can manage a varied caseload effectively.
  • Has excellent communication and organisational skills.
  • Is compassionate, patient and committed to achieving positive outcomes.
