Are you an experienced Children's Social Worker with a passion for supporting children and young people with disabilities?

We are currently looking for a dedicated and compassionate Children with Disabilities Social Worker to join a supportive children's services team, helping children and young people with disabilities achieve greater independence, wellbeing and positive outcomes.

The Role

You will work with children and young people with a range of disabilities and additional needs, supporting them and their families to access the right services, care and support.

Your responsibilities will include:

  • Completing assessments of children and young people's care and support needs.
  • Developing and reviewing person-centred care and support plans.
  • Undertaking statutory visits and reviews where required.
  • Working directly with children and young people to understand their wishes, feelings and needs.
  • Supporting families and carers to access appropriate services and resources.
  • Coordinating packages of care and support.
  • Working closely with schools, health professionals, short-break services and other agencies.
  • Working with families around transition planning into adulthood.
  • Identifying and responding appropriately to safeguarding concerns.
  • Supporting children and young people to maximise their independence and participate in their communities.
  • Attending meetings, reviews and multi-agency discussions.
  • Maintaining accurate and up-to-date case records and reports.

About You

We are looking for a Social Worker who:

  • Holds a recognised Social Work qualification.
  • Is registered with Social Work England.
  • Has experience working with children and young people with disabilities and/or additional needs.
  • Has experience completing assessments, reviews and care planning.
  • Has a good understanding of safeguarding and children's social care legislation.
  • Understands the needs of children with disabilities and their families.
  • Is confident working with families, carers and multidisciplinary professionals.
  • Has excellent communication and organisational skills.
  • Can manage a varied caseload effectively.
  • Is compassionate, patient and committed to achieving positive outcomes for children and young people.

DfE Requirements

Candidates must meet the relevant Department for Education (DfE) requirements for the role, including the required social work qualification, professional registration and relevant experience.
