We have a forthcoming permanent vacancy for a Practice Nurse to work two days per week (three days negotiable). The role involves delivering practice nursing services within a multidisciplinary team, focusing on infection prevention, health promotion, chronic disease management, and supporting clinical policy review.

Key Responsibilities:
- Provide nursing services to the practice population, including chronic disease treatment, screening, and advice.
- Conduct treatment room sessions, manage patients, arrange follow-up, liaise with other team members.
- Administer injections, immunisations, ear syringing, dressings, minor injuries.
- Assist medical colleagues with minor surgical procedures and contraceptive device fitting.
- Telephone advice, referrals, chaperone examinations as required.
- Perform phlebotomy and venepuncture as needed.
- Lead chronic disease management and health promotion sessions.
- Maintain infection control measures, stock checks, health & safety.
- Support clinical governance, training, mentorship.
Person Specification:
Experience (Essential):
- Working in accordance with the NMC Code of Professional Conduct.
- Proficiency with Microsoft Office and clinical system software.
- Public/patient engagement experience.
- Multi-skilled chronic disease management approach.
- Demonstrated performance on the Quality & Outcomes Framework.
Desirable:
- Diagnosis and treatment of minor illnesses.
