Overview

An exciting opportunity has arisen for highly motivated individuals with at least 2 years recent renal/acute experience to join the Chronic Kidney Disease Team.

Responsibilities

As a Renal Specialist Nurse working within a nursing team you will be responsible for caring for patients with progressive chronic kidney disease, preparing them with individualized care and education for renal replacement therapies, transplantation or conservative management.

You will assist in the management of patients with all aspects of chronic kidney disease, coordinate care for patients with CKD 4 to 5, prepare and educate them for dialysis or conservative management, be involved in anaemia management for patients with CKD, participate in clinics off site at Walsall and Cannock, either nurseโ€‘led or as part of the MDT, and expected to do home visits to patients as required. You will work closely with the Transplant Team and assist in transplant education and workup.

Work independently, manage your own caseload of patients, and dictate clinic review letters and make referrals to other departments and services when necessary.

Qualifications

Competent/confident individual with ability to work autonomously. Excellent communicator and possess excellent organisational skills.

Evidence of relevant post registration education. The V300 prescribing prescribing qualification is desirable. Otherwise evidence that you are working towards it.

Working Hours & Flexibility

The department is open Monday to Friday 8am to 5pm, closed on weekends and bank holidays.

The advertised role is full time, with some flexibility within departmental operating hours. Part time working may be considered if service needs are met.
