In this role you will be responsible for proactively supporting effective patient flow and supported discharges - both at ward level and with the hospital's Integrated Discharge Service. It's a great opportunity to make a positive impact as you will be working closely with colleagues in the Integrated Discharge Service, the Care Transfer Hubs and Clinical Site Team to help patients get home to continue their recovery.

You will work within both a designated ward team and with the Integrated Discharge Service and Clinical Site Team providing support to members of the teams. The Integrated Discharge Service is responsible for supporting wards with complex discharges across the RUH and they lead the RUH Care Transfer Hubs which is where all discharge referrals are triaged, and discharge pathways agreed alongside community partners and multi-disciplinary teams. The Clinical Site Team coordinates the management of inpatient bed capacity to match demand.

The ethos of the Integrated Discharge Service is to identify patients at ward level who require a supported discharge, working with patients, families and ward staff whilst patients are not medically stable by planning discharge arrangements and discharging patients as soon as possible once medically stable. If you are a team player and enjoy joint working with other providers and community teams, you will be supporting us to provide a β€œwhy not home, why not today” approach to reduce length of stay and facilitate early discharge.

Main duties of the job:
  • Proactive progression of timely and safe patient flow and discharge, minimising avoidable delays, ensuring and maintaining accuracy of information, supporting the ward team to progress chase.
  • Working within a cluster of wards and being prepared to assist other wards as workloads require, ensuring that flow is optimised at all times.
  • Communicating with patients and carers as required.
  • Facilitating the smooth communication of discharge information between wards and community providers.
  • Active listening skills and the ability to document accurately.
  • An awareness of hospital pressures relating to flow and discharge.
  • Ensuring that patient information, including that of a highly confidential nature, is shared appropriately and meets Information Governance standards.
  • To have an in-depth understanding of the processes required for ward discharges.
  • Supporting the administrative role of the ward staff.
  • Develop working relationships with key people or teams, such as the Community Hubs Care Coordination Centre, Discharge Liaison Practitioners, Discharge Lounge, Ward Managers, Therapy team leads, and other ward flow and discharge coordinators.

Person specification:
  • Essential criteria: Education to GCSE standard to include English, or equivalent; Knowledge of patient administration systems; IT Literate, confidence and experience working across Teams/Excel, Word.
  • Desirable criteria: NVQ Level 3 or working towards this; Experience within the NHS or similar healthcare setting.
