We have an exciting opportunity to join our friendly and supportive Clinical Hub at Trinity as a First Contact Clinical Nurse. The main purpose of this role is to follow patient referrals, undertake holistic telephone assessments of patients, their families and/or carers to identify their immediate and anticipated specialist palliative care needs.

Main duties of the job:

  • To play a key role in receiving and managing referrals made to the hospice.
  • To ensure a timely, responsive, and effective service for the benefit of all stakeholders involved in the patient referral.
  • Manage all aspects of the referral system to the hospice identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them.
  • Use assessments and interventions as opportunities to train, coach and mentor clinical staff and visitors to the hospice about specialist palliative care practice.
