We are looking for an experienced Triage Clinical Nurse Specialist with proven ability to implement and oversee triage systems and protocols to ensure safe, timely, and effective patient flow. Knowledge in Clinical Triage in line with NHS Pathways and Advanced Telephone Triage. This post requires you to act as the first point of clinical contact for the patient, following patient referral. They will identify and respond to the immediate specialist palliative care needs, through direct liaison with the patient, carer or other involved health and social care professionals.

Key responsibilities

  • To play a lead role in receiving and managing referrals made to the hospice.
  • Triage all incoming referrals considering patient’s stated symptoms to determine the appropriate level of care required to safely meet the patient’s specialist palliative care needs.
  • Utilise agreed resources and guidelines to effectively assess, prioritise, advise, schedule patient care management or refer patient when necessary to other appropriate health and social care agencies.
  • To ensure a timely, responsive, supportive and effective service for the benefit of all stakeholders involved in the patient referrals.
  • Manage all aspects of the referral system to the hospice – identifying appropriateness, urgency, and level of priority.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice, and ensure care is planned to meet the individualised and dynamic needs of the patients, and those close to them.
  • Use assessments and interventions as opportunities to train, coach and mentor clinical staff and visitors to the hospice about specialist palliative care practice.

Skills and experience required

  • Registered Nurse with bachelor’s degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.
  • Advanced assessment skills.
  • Critical Thinking Skills.
  • Able to demonstrate knowledge of disease trajectories and treatment effects.
  • Advanced Communication skills.
  • Enthusiasm and a flexible attitude to working.
  • Ability to work autonomously and as part of a team.
  • The successful candidate must be able to travel with ease across our catchment area (a current UK driving licence is desirable).
