Job summary
It is an exciting time to join this team, as we work to implement our Ageing Well and Frailty Strategy across the Guilford and Waverley healthcare system. Opportunities exist to rotate through all of these pathways and prospective cover is within budget. The MICH hub was developed during the pandemic and looks to support neighbourhood teams with standardised CGA assessment. We are looking to work with system partners to risk stratify older people living with frailty, to proactively offer CGA to them and to integrate with locality based MDTs.
All new posts will contribute to our Frailty Academy which has been running since 2021 and has embedded frailty training across the organisation and local system, including the development of the a trainee ACP programme and an internal CESR programme. We recently won a bid to expand the academy and to help develop a strategic workforce plan fit to meet the needs of an ageing population.
