We are seeking a motivated and compassionate individual to join our team as a Community Advocate. The successfulcandidate will play a crucial role in supporting adults across Hammersmith & Fulham, helping them understand their
rights, make informed choices, access services and have greater choice and control over their lives.
Key Responsibilities:
- Provide one-to-one advocacy support to adults with disabilities, mental health conditions and complex health needs.
- Empower individuals to understand their rights, options and choices and encourage self-advocacy wherever possible.
- Support clients to challenge services and decisions where necessary and access appropriate support.
- Meet clients in the office, their homes and community settings, and accompany them to appointments and meetings.
- Work collaboratively with social services, Libra Partnership and other professionals to remove barriers and increase opportunities.
- Manage an advocacy caseload of up to 21 people, ensuring cases progress and close within appropriate timeframes.
- Promote equality and inclusion, challenge discrimination and respond appropriately to safeguarding concerns.
- Support accessible communication using signs, symbols, photos, multimedia and other appropriate resources.
- Help people access housing, benefits, health and wellbeing services, community activities and opportunities through Individual Budgets and Self-Directed Support.
- Maintain accurate case records, contribute to monitoring and reporting, attend supervision and training,and work in line with HF Mencap's policies and procedures.
