Stratford Central PCN is looking for an enthusiastic and proactive Care Coordinator to join the team and deliver care to patient populations. This new and evolving role within NHS Primary Care Networks will involve flexible work, combining focussed project work with core support to the PCN. Care Coordinators provide extra time, capacity and expertise to support patients in preparing for or following up on clinical conversations with primary care professionals. They support Clinical Directors in coordinating key activity, improving access to services, providing advice and information, and ensuring health and care planning is timely, efficient and patient‑centred. They work closely with Social Prescribing Link Workers and support delivery of the comprehensive model of care reflecting local priorities, health inequalities and population health management.

Location and travel

The successful candidate will be required to travel between Stratford PCN practices. A clean UK driving licence and full use of a car are essential.

Key responsibilities

  • Coordinate key activities and improve access to services.
  • Provide advice and information for patients and clinical staff.
  • Ensure timely, efficient and patient‑centred health and care planning.
  • Work in partnership with Social Prescribing Link Workers to support the comprehensive care model.

Qualifications

Essential:

  • Good experience of IT systems and packages including EMIS Web and Docman 10.
  • Experience of data collection and providing monitoring information.
  • Experience of supporting people, their families and carers in a related role.
  • Demonstrable commitment to professional and personal development.
  • Meets DBS standards and has a clear criminal record, in line with the law on spent convictions.
  • Access to own transport and ability to travel across the locality on a regular basis, including to visit people in their own homes.

Desirable:

  • Experience of working directly in a community development context, adult health and social care, learning support or public health/health improvement (including unpaid work).
  • Experience of supporting people with their mental health.
  • Training in motivational coaching and interviewing or equivalent experience.
