The Band 7 Community Nursing Clinical Lead provides visible, clinically focused leadership to community nursing teams, including Community Matrons, to ensure safe, effective and person-centred care. The post holder works autonomously and is responsible for oversight of clinical caseloads, triage, complex assessment, care planning, direct patient care and escalation of clinical risk. They support safer staffing, rostering, supervision, appraisal, workforce development and staff wellbeing, while maintaining professional, safeguarding and clinical governance standards.
The role includes leading audits, incident and complaint investigations, quality improvement and service development, alongside monitoring performance and the effective use of resources. Working collaboratively with operational managers, multidisciplinary colleagues, primary care, partner agencies, patients and carers, the Clinical Lead promotes integrated neighbourhood working, evidence-based practice, high-quality documentation and continuous learning. Approximately 80% of the role is clinically focused, with 20% allocated to non-clinical leadership and management responsibilities.
Clinical Leads roles focus on the case management of the patients within the service, ensuring the right care, for the right time and with the right clinician. In addition, the role has an enabling focus on the knowledge, skills, competence and confidence of the teams by working alongside the teams and the patient. This role is perfect for those leaders that want to maintain direct clinical contact with the patient whilst also be given the opportunity to drive clinical change through excellence. The Clinical Lead role is a locality based role, with the base of Honiton. The role includes 8-8 on a roster basis alongside other leads in the locality. The role will include participation in locality triage.
Working Pattern: 37.5 Hours Participation in an 8-8, 7 day working on a locality roster.
