This role is open to all applicants. However, in line with Cornwall First recruitment principles, initial shortlisting will prioritise applicants from Royal Cornwall Hospitals NHS Trust, Cornwall Partnership NHS Foundation Trust, and the Integrated Care Board for Cornwall and the Isles of Scilly.

An exciting opportunity has arisen to support in the development and provide a clinical pharmacy service to emergency medicine at RCHT.

To provide specialist pharmacy advice and training in medicines management and optimisation to consultants & junior medical staff, nursing staff, pharmacy staff, other healthcare professionals, & patients/carers.

To assist in the provision of regular structured feedback to emergency care regarding the safe and effective use of medicines and drug expenditure to facilitate service improvement.

Undertake and contribute to clinical audit/quality improvement projects within Pharmacy and emergency care.

To deputise for lead pharmacist for emergency medicine.

Main duties of the job:

  • Support the lead pharmacist for emergency care in the development of clinical pharmacy services.
  • Providing core clinical pharmacy service to emergency medicine in line with the Trust and clinical pharmacy standards.
  • Ensuring high quality prescribing and care is provided on and during admission through to discharge to patients by effective clinical screening.
  • Ensuring that pharmaceutical knowledge and skills are applied through medication reconciliation on admission, optimisation during their admission and input to planning for a successful discharge process where appropriate.
  • Identify medicine-related causes for admission and develop treatment plans to optimise care.
