Main duties of the job:
- Train patients to undertake home dialysis treatments both in haemodialysis and peritoneal dialysis.
- Aim to increase uptake in home dialysis as the first choice treatment option by delivering group education sessions regularly.
- Visit patients in their homes where indicated.

- Independently manage a caseload and time management to respond promptly to emerging clinical needs and ensure optimal delivery of care within the modality.
