We have an exciting opportunity for Social Workers to join the Community Team!

These are full-time, permanent roles with hybrid working, and a salary of £38,192 – £42,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

About The Team

The Community Team is comprised of Senior Social Workers, Social Workers and Social Care Practitioners who all undertake care needs assessments and carers assessments, as well as mental capacity assessments. They participate in multi-disciplinary team settings with the aim of achieving comprehensive and holistic assessments. Each month they hold a team meeting to analyse and review our ways of working. For the benefit of practitioners, we also hold practice sessions whereby we discuss complex cases using objective approaches and sharing experiences.

The Role

As a Social Worker, you will be at the forefront of empowering adults to live independent and fulfilling lives. You will manage an allocated caseload, developing care plans and providing proportionate interventions in line with the Care Act. Your work will involve undertaking holistic, person-centred assessments and reviews to help residents achieve their personal goals and promoting their independence. You\’ll be responsible for responding to and raising safeguarding concerns, while also fulfilling key statutory duties under the Care Act, Mental Capacity Act, and Mental Health Act.

What We Are Looking For

  • Qualified Social Worker with current registration with Social Work England.
  • BA, BSc or equivalent qualification.
  • Diploma qualification in Social Work.
  • Experience of working in a health or social care environment.
  • Strong understanding of relevant legislation, including the Care Act, Mental Capacity Act and safeguarding protocols.
  • Excellent communication, negotiation, and time management skills.
