Role Overview

You will report to the Service Team Manager / AMHP Lead and work alongside the psychiatric inpatient liaison to carry out Care Act assessments, coordinate community resources, and facilitate safe, timely discharge.

Responsibilities

  • Manage workload and facilitate hospital discharge in a timely manner.
  • Promote multi‑agency working with health and social care colleagues.
  • Undertake Care Act assessments and review care packages and placements in line with the Care Act 2014.
  • Deliver person‑centred service, adopting a strength‑based approach to promote independent living in the community.

Qualifications and Experience

  • Experience delivering statutory adult social care services or working with vulnerable adults who have mental health conditions.
  • Experience working with people, preferably those who are vulnerable or have disabilities.
  • Knowledge of relevant legislation.
  • Understanding and application of the principles of person‑centred care.
  • Ability to work within one's skill level, identify learning needs and seek support appropriately.
  • Good written and oral communication skills, including report writing.
