We have an exciting opportunity for a Social Care Practitioner to join our Hospital Team in Maidenhead. This full‑time permanent role is based in the Town Hall, offers hybrid working and a salary of £26,895‑£30,132 per annum.
About the Role
As an Adult Social Care Practitioner you will support residents in their own homes, hospitals and care settings by assessing their needs and coordinating tailored support that promotes independence and wellbeing.
Your Role Will Involve:
- Assessing and supporting adults with care needs in line with the Care Act 2014.
- Managing a caseload, working collaboratively with individuals, families and professionals.
- Coordinating services that promote independence, safeguarding and risk management.
- Ensuring compliance with legislation, policies and budgetary frameworks.
- Maintaining accurate records, contributing to team performance and upholding the council’s values and governance standards.
