The Role:

Joining our Independent Futures Team as a Senior Case Worker is an excellent next step in your career in Social Work. You’ll be an integral part of our team providing support to young people with disabilities as they transition from Children’s Services to Adults’ Health and Care.

As well as managing your own diverse caseload, you’ll support cases with higher complexity levels and provide guidance to colleagues, role modelling best practices and setting high standards across the team.

What you’ll do:

  • Complete assessments and reviews of individuals in the community.
  • Provide support and guidance to maximise wellbeing and independence.
  • Collaborate with individuals, families, providers, and healthcare professionals to find and signpost to community resources.
  • Make well-informed decisions and escalate cases when necessary.

What we’re looking for:

  • Strong communication and relationship management skills.
  • Ability to work compassionately in difficult circumstances.
  • Strong analytical and problem-solving skills.
  • Good IT and record-keeping skills; ability to work independently and as part of a team.
  • Understanding of the Care Act 2014 and experience in adults’ social care.
