About the role:

Health is a positive state of wellbeing rather than simply the absence of disease, and as such is determined more by socio-economic factors and degree of social capital than medical conditions. Despite this, health services primarily focus on providing medical management of illnesses rather than addressing the underlying social determinants of health to create wellness. Social prescribing changes this by helping people who use health services to take control of their circumstances. Whether it relates to debt, housing, or relationships, it helps them to become more confident in navigating their issues, and more connected with and more actively involved in what is already going on in their local community.

LBHF has partnered with North H&F Primary Care Network to deliver a social prescribing service from all 9 of the GP practices in the W12 area. As one of our team of ‘link workers’ you will be the bridge between the GP practice, the local community, and the Local Authority. In this role you will work to build trusting relationships between organizations and create a shared personalised care and support plan for patients who have been referred. You will support existing groups and help establish new community groups, working collaboratively with all local partners. This link worker role is about people and places, so we are looking for enthusiastic, motivated, and energetic individuals who have a passion for people and want to make a difference in the North Hammersmith & Fulham community.
