The Royal Borough of Kensington and Chelsea is seeking a qualified social worker to lead hospital discharge planning across our local boroughs.
Based in the Hospital Adult Social Care Team, you will assess needs, coordinate care and help residents return home safely.
You’ll work within a fast-paced, multi-disciplinary environment, collaborating with families, carers, NHS colleagues and other professionals to ensure support plans are in place and risks are managed effectively.
