An opportunity has arisen for an experienced Social Work practitioner with experience and interest in working with individuals with complex Neurological conditions, including Motor Neurone Disease, Multiple Sclerosis, Parkinsons Disease, Huntingdon’s Disease, Stroke and Brain and Spinal cord Injuries, among others.
The post is offered initially as a one-year secondment, subject to evaluation, as part of a pilot project, aiming to enhance holistic care by addressing the social, financial, emotional and practical needs, and aiming to achieve improved outcomes including more tailored assessments and designated support, reducing hospital admissions, streamlining integrated care through strengthened communication and co‑ordination between health and social care professionals, and working collaboratively to advocate for and empower patients and carers.
The post-holder will be based between the Neuro‑Rehabilitation Multidisciplinary Team at the Memorial Hospital (managed within Oxleas NHS Foundation Trust), and the Complex Care Team in Adult Social care at the Woolwich Centre, within The Royal Borough of Greenwich. Clinical supervision will be facilitated by the Social Work manager with line management support within the Neuro MDT, with access to training and development across the two organisations.
Candidates need to be a Registered Social Worker with a minimum of two years post‑qualification experience, and demonstrate previous experience of, and / or a keen interest in, working with those with complex neurological needs. A significant knowledge of social care and health legislation and policy essential.
