Silverstone Primary School is recruiting a Senior Social Work Practitioner to join the Joint Emergency Team (JET) within the Royal Borough of Greenwich. This permanent, full‑time role (35 hours per week) includes an out‑of‑hours component and offers a base salary of £49,056 to £62,918, inclusive of out‑of‑hours enhancements.
Overview
The JET team delivers a proactive, community‑based hospital admission avoidance service that brings together a multidisciplinary group to manage health and social care crisis interventions. The team includes GPs, nurses, occupational therapists, physiotherapists, social workers, pharmacists and a handyman, working collaboratively to provide a comprehensive wrap‑around service.
Key Responsibilities
- Supervise and support social work staff, participating daily in workflow and quality assurance of Care Act assessments.
- Implement best practice and adult safeguarding principles, acting as Adult Safeguarding Manager.
- Lead a small caseload, providing advice and guidance on complex cases, and working in partnership with other professionals.
- Lead social work staff during out‑of‑hours discharges and community referrals, working one weekend in four and one or two late shifts (12:30 pm – 8:30 pm) per week.
- Participate in the Out‑of‑Hours on‑call service (8:30 pm – 8:00 am, Friday to Friday, once a month).
- Promote a strengths‑based practice culture and drive continuous professional development for staff.
