ADAPT provides a range of support to working age adults aged 16-65 with mental health problems, including complex symptoms of depression, anxiety, complex psychological difficulties including personality disorder and complex psychosocial needs.

You will be part of a multi-disciplinary service that consists of team manager, Community Mental Health Nurses, Mental Health Social Workers, Occupational Therapists, Community Support Workers, Admin Support, Secondary Care Psychological Therapies and Consultant Psychiatrists.

The Social Worker will be responsible for Care Act assessments, reviews, safeguarding enquiries, and mental capacity act assessments. The successful candidate will work as part of a strong and dynamic social work service and will be required to undertake strengths-based assessments applying relevant legislation, utilizing social work skills and knowledge to develop and implement a plan of support aimed at improving wellbeing and delivering outcomes for people.

The Social Worker will manage a caseload collaborating with users and family or friend/carers, undertake strengths-based assessments and reviews, identifying eligible needs and personalised outcomes.

What we look for:

  • All social workers will be expected to be able to manage a caseload and to work co-productively with service users, their families and carers to ensure that a strength-based, person-centred approach remains at the core of all interventions.
  • To be part of a multidisciplinary team that consists of social workers, occupational therapists, psychiatrists, psychologists, support workers, lived experienced practitioners, community psychiatric nurses, and administrative support.
  • To work autonomously with individuals, carers, and families to deliver a range of assessment and care interventions that promote mental health wellbeing and strengths-based personalised outcomes for people, whilst promoting independence.

You will be expected to undertake statutory functions of the social work role (Care Act 2014, Mental Capacity Act 2005, Mental Health Act, Children Act) and other related legislation, responding to requests for those who present with care and support needs:

Senior Social Work/AMHP Role and Responsibilities (applicants with AMHP Qualification):

  • Use their social care knowledge and skills to guide intervention with service users, families, and carers.
  • Undertake care act assessments, support planning, and reviews under the Care Act 2014.
  • Undertake the AMHP role on behalf of the local authority and participate in the Greenwich Central AMHP Team borough wide AMHP rota (pro rata).
  • Undertake and provide consultation and support on Safeguarding Adults and Children procedures and to act as a Safeguarding Adults Manager, Enquiry Officer, MARAC Representative and Think Family Champion where appropriate.
  • Undertake work with individuals and their families, to develop and maintain self-help, improve problem solving and coping capabilities towards improved health and wellbeing.
  • Participate in Section 117 discharge and care planning processes and meetings.
  • Promote and undertake interagency collaboration and networking with the local authority, housing, third sector organisations, voluntary agencies, and community networks to mobilise resources.
  • Complete statutory documentation and reports within the required time scales.
  • Assess the needs of groups who are difficult to engage or those from excluded groups (i.e. nil recourse, drugs/alcohol).
  • Undertake mandatory and relevant training to maintain their social care practice, knowledge, and skills.
