Join us at Routes Healthcare as a Healthcare Assessor. Your pay will be based on your skills and experience, up to £27.5k inclusive of on‑call and bonus.

About the Role
Do you take pride in delivering high‑quality care to people in your community? As a Healthcare Assessor you will create person‑centred care plans, ensuring patients receive the care and support they need in line with CQC standards.

Key Qualifications
- Passionate about putting people first
- Minimum 1 year experience in community care
- Strong organisational and time‑management skills
- Good understanding of CQC regulations
- Excellent communication skills, able to build relationships with clients and care teams
- NVQ in Health and Social Care is a plus
- Full valid UK driving licence and access to own vehicle
Typical Day‑to‑Day
- Developing care plans and risk assessments
- Overseeing safe administration of medicines
- Undertaking observations, spot checks and quality assurance
- Completing and updating care records on in‑house systems
- Assisting the Registered Manager to deliver high‑quality care
- Managing on‑call duties on a rotating basis
