We're looking for a Children's Nurse Case Manager to join Routes Healthcare's specialist complex care team on a full-time, permanent basis. You'll take clinical ownership of a caseload of children with complex needs — leading assessments, overseeing care delivery, supporting frontline staff, and acting as the key clinical point of contact for families, commissioners, and healthcare professionals. This is a community-based, travelling role across our network in the North of England. We're ideally looking for someone based along the M62 corridor.

What the role involves:
- Clinical case management of children with complex care needs across the Routes network
- Conducting and reviewing risk assessments, care plans, and clinical documentation
- Providing clinical supervision and competency sign-off for care staff
- Acting as the named clinical lead for your caseload — liaising with families, GPs, consultants, community nursing teams, and commissioners
- Supporting new package set-up, including initial assessments and staff induction
- Responding to clinical escalations and supporting incident review
- Contributing to audits, quality assurance, and CQC compliance
Clinical interventions you'll have working knowledge of:
- Tracheostomy care and management
- Ventilation (invasive and non-invasive)
- PEG feeding and enteral nutrition
- Oxygen therapy and airway management
- Complex medication management
What we're looking for:
- Registered Children's Nurse (RCN) with a valid NMC pin
- Proven experience in tracheostomy and ventilation care — essential
- Previous case management, community nursing, or clinical leadership experience — highly desirable
- Confident communicator with families, clinical teams, and commissioners
- Based along the M62 corridor with a full UK driving licence and access to a vehicle
- Organised, accountable, and able to manage a varied caseload independently
