Are you a Registered Children's Nurse ready to step into a clinical leadership role? Do you have experience managing complex care packages and the confidence to lead clinical oversight across a specialist caseload? If so, this could be the role you've been waiting for.

We're looking for a Children's Nurse Case Manager to join Routes Healthcare's specialist complex care team on a full-time, permanent basis. You'll take clinical ownership of a caseload of children with complex needs β€” leading assessments, overseeing care delivery, supporting frontline staff, and acting as the key clinical point of contact for families, commissioners, and healthcare professionals.

This is a community-based, travelling role across our network in the North of England. We're ideally looking for someone based along the M62 corridor.

A little about us: At Routes, we're more than just a healthcare provider. We're a dedicated team that genuinely cares about our clients, and about each other. For over 15 years, we've delivered care that supports people to live how they choose, independently, safely, and with dignity in their own homes.

If you're a clinician who wants to lead, influence, and make a systemic difference, not just at the bedside, you're already our kind of person.

What you can expect from us:

  • A senior clinical role with real autonomy and responsibility
  • Supportive leadership and a collaborative multidisciplinary network
  • Access to Routes Academy for continued professional development
  • Digital systems for care planning and case management
  • A varied and meaningful caseload across complex paediatric packages
  • Zero agency reliance β€” we invest in our own team

Some perks to sweeten the deal:

  • Salary up to Β£50,000 per annum
  • Clinical training updates
  • Access to our Employee Assistance Programme (EAP)
  • Mileage to and from clients
  • Buy and sell holiday scheme

What the role involves:

  • Clinical case management of children with complex care needs across the Routes network
  • Conducting and reviewing risk assessments, care plans, and clinical documentation
  • Providing clinical supervision and competency sign-off for care staff
  • Acting as the named clinical lead for your caseload β€” liaising with families, GPs, consultants, community nursing teams, and commissioners
  • Supporting new package set-up, including initial assessments and staff induction
  • Responding to clinical escalations and supporting incident review
  • Contributing to audits, quality assurance, and CQC compliance

Clinical interventions you'll have working knowledge of:

  • Tracheostomy care and management
  • Ventilation (invasive and non-invasive)
  • PEG feeding and enteral nutrition
  • Oxygen therapy and airway management
  • Complex medication management

What we're looking for:

  • Registered Children's Nurse (RCN) with a valid NMC pin
  • Proven experience in tracheostomy and ventilation care β€” essential
  • Previous case management, community nursing, or clinical leadership experience β€” highly desirable
  • Confident communicator with families, clinical teams, and commissioners
  • Based along the M62 corridor with a full UK driving licence and access to a vehicle
  • Organised, accountable, and able to manage a varied caseload independently

This is a senior clinical role with genuine scope β€” to lead, develop, and make a lasting difference to children and families across the North of England, backed by a team that takes complex care seriously.
