Salary: Up to £40,000 depending on experience

Contract: Full-Time, Permanent

An exciting opportunity has arisen for a Nurse Case Manager to join Routes Healthcare, where you\’ll play a pivotal role in supporting and overseeing complex care packages maintaining the highest standards of clinical excellence and facilitating truly person-centred care.
