Routes Healthcare on behalf of Just A Little Company is seeking a Nurse Case Manager. The salary is up to £45,000 depending on experience, with a full-time, permanent contract from Monday to Friday, including some ad hoc flexibility for evenings and weekends.
We have an exciting opportunity available for a Nurse Case Manager, where you will play a pivotal role in supporting and overseeing our complex care packages, maintaining best quality and clinical excellence to facilitate person-centred care.

Your responsibilities will include:
- Management of complex care packages, both clinically and holistically.
- Developing, reviewing and implementing care plans and risk assessments.
- Managing and delivering the training needs of the care team, maintaining an appropriate skill mix to ensure safe and effective clinical practice.
- Engaging in networking, attending MDT meetings and working with clinical commissioning groups.
- Conducting new and ongoing competency assessments with the care staff team.
- Reviewing care packages.
Requirements:
- A valid NMC pin.
- Experience in a high dependency or critical care setting would be advantageous.
- Experience in tracheostomy and ventilation management in a community setting is desirable.
- Computer literate with experience of using Word, Excel and Outlook.
- Flexible and autonomous.
- An understanding of CQC regulations.
- Access to a car for work.
