We are seeking a driven, emotionally intelligent, high quality Advanced Nurse Practitioner (ANP) who values excellent patient care to join us. Main duties of the job:

  • Your role will be to assess patients via telephone triage and face to face consultations.
  • You will be able to complete an assessment, diagnosis, treatment, prescribing and referral as appropriate.
  • Practice autonomously, are accountable and responsible for decision, actions and omissions and are self-directed.
  • Proactively identify, diagnose and manage treatment plans for patients at risk of developing long-term condition(s) (as appropriate).
  • Assess individuals, families and populations holistically using a range of different person centred approaches.
  • Plan and manage complete episodes of care, working in partnership with others, and delegating and referring as appropriate to optimise health outcomes and resource.
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs.
  • Maintain accurate patient records and enter onto the computer using agreed templates, Read Codes, manage patient results and paperwork which is assigned to you.
  • Maintain own 3 yearly registration with the NMC.
  • Attend training courses/study days as deemed useful or necessary to maintain own continuous professional development and ensure PDP kept up-to-date in line with the requirements of the Practice.
  • Attend and participate in staff meetings as appropriate.

Roundwood Surgery operates from a purpose-built surgery in central Mansfield and benefits from a smaller branch surgery in Forest Town. List size of 13600 patients. Training Practice with up to 4 registrars and two F2 trainees. One Advanced Nurse Practitioner, 4 Practice Nurses (1 Nurse Prescriber), 2 Health Care Assistants and a Phlebotomist. A full complement of experienced reception and administration staff. Primary Care Network support of 5 Social Prescribers, 2 WTE pharmacists, CPN Nurse, Community Practice Nurse, Nurse Associate and Specialist Nurse Support for Learning Disabilities. Q.O.F. - high achieving. Use of SystmOne software. Being part of our friendly approachable team.

Job responsibilities:

  • Provide direct access to patients seeking health care management and advice.
  • The successful applicant will be an Independent Advanced Nurse Practitioner who can work within the legal framework of nurse prescribing and local prescribing policies.
  • Ability to formulate a diagnosis using medical diagnostic problem solving skills.
  • Undertake a thorough history, physical examination, invasive and non-invasive procedures/investigations.
  • Initiate appropriate diagnostic tests including radiology, imaging and laboratory tests with attention to safety, cost effectiveness, acceptability and efficiency.
  • Work in a collaborative manner with other agencies to ensure seamless service delivery for patients.
  • Assess, diagnose, treat or refer patients as an autonomous primary care practitioner.
  • Request pathology/radiology investigations and be able to process/manage these results.
  • Apply educational knowledge and skills to support the development of other members of the clinical team.
  • Demonstrate evidence-based practice in line with clinical governance principles and to provide clinically effective care.
  • Provide health promotion advice on an individual basis.
  • Demonstrate an awareness of other community and support resources available and provide necessary information to the patient and carry out required referrals.
  • Demonstrate awareness of cultural or language barriers to health and take action to mitigate these.
  • Ensure own clinical competence and updating through ongoing professional development and the active participation in clinical and staff meetings.
  • Provide professional and clinical support and advice to other members of the primary healthcare team.
  • Keep professionally updated and be responsible for addressing any unmet personal training needs.

Person Specification:

Experience:

  • Experience of working in a primary care environment.
  • Experience of prescribing and undertaking medication reviews.
  • Experience of working as a practice nurse.

Qualifications:

  • Registered Nurse.
  • Post graduate diploma or Masters degree (Advanced Practice Qualification).
  • Independent Prescriber (extended and supplementary).
  • Qualified Triage Nurse.

  • Evidence research and development.

Analytical & Judgemental Skills:

  • Able to understand highly complex situations/information e.g. working with patients with acute, non-acute and chronic illnesses and using a range of skills to decide the best course of action from a range of options and consider the implications of each of these.
  • Be able to manage clinical uncertainty.
  • Be able to manage conflict.

Knowledge and Experience:

  • Minimum of 3 years NP experience.
  • Telephone triage.
  • Able to manage acute illnesses, long term conditions and minor ailments.
  • Refer patients independently as appropriate.
  • Arrange appropriate investigations.
  • Interprets a range of diagnostic tests and routine clinical procedures.
  • Studies experience within the clinical field.
  • Demonstrate the application of evidence based practice.
  • Able to demonstrate a depth and breadth of current issues and policies within the NHS and have the ability to translate into practice.
  • Experience in leadership role and change management.
  • Autonomous practitioner working independently and in conjunction with other healthcare professionals.
  • High level of clinical skills and competency relevant to the role.
  • Understanding and experience of interagency working and adult/child protection.
  • Evidence of continued professional development.
  • Experience of Clinical Governance and risk assessment.
  • Experience in operating a recall system in general practice.
  • Evidence of teaching/mentoring students/groups.
  • Demonstrate high level of self-awareness i.e. strengths and weaknesses, personal qualities and skills.
  • Able to communicate with a range of people at all levels of the organisation.
  • Demonstrate leadership behaviour in a variety of settings i.e. strategically and corporately.
