We are recruiting for a Leaving Care Worker to support young people aged 16+ transitioning from care to independent living within supported accommodation and community-based settings. This role involves delivering person-centred support to help young people develop practical life skills, improve wellbeing, access education, training, employment, and maintain suitable accommodation while preparing for adulthood.
Key Responsibilities
- Provide direct support to young people aged 16+ transitioning from care to independence.
- Deliver 1-1 support, keyworking sessions, and independent living training.
- Develop, implement, and review support plans in partnership with young people, families, social workers, and professionals.
- Support young people with life skills, budgeting, tenancy readiness, education, employment, training, and community engagement.
- Maintain accurate records, reports, case notes, and documentation in line with policies and regulatory standards.
- Follow support plans, risk assessments, behaviour management plans, and safeguarding procedures.
- Build positive working relationships with housing providers, education providers, employers, local authorities, and external agencies.
- Promote participation, wellbeing, social inclusion, and positive outcomes for young people.
- Work flexibly across different settings including evenings, weekends, and sleep-in shifts.
