About the role

Do you enjoy working in a busy environment, working as part of a team with lots of variety to your day? If so, being an Assessment and Review Coordinator in the IDT- Transfer of Care Hub could be the role for you. An exciting opportunity is available within Adult Care, Housing and Public Health, as an Assessment and Review Coordinator working in our Integrated Discharge Team (IDT)- Transfer of Care Hub. The post is based within the Transfer of Care Hub of Adult Social Care where the focus is to ensure that adults of Rotherham and their carers are enabled to prioritise their well-being, maximise assets, promote independence and ensure vulnerable adults are safeguarded appropriately when needed.

As an Assessment and Review Coordinator you will be responsible for screening referrals and enquiries over the telephone and providing a response to our customers and families to facilitate safe discharges from hospital, Intermediate Care unit and to prevent hospital admissions. Working as part of a multi-disciplinary team to undertake statutory assessments under the Care Act (2014). You will support customers to maximise their independence and personal outcomes through the provision of comprehensive advice, information, practical support, and signposting. Focusing on strengths, prevention, early intervention, and the promotion of wellbeing throughout your practice. You will ensure that customers receive the right support, advice, and information at the time they need it.

You may be completing referrals to access short term support or assistive technology to help people live independently in their own homes. You will work as part of a wider team and work closely with Social Workers, Occupational Therapy colleagues and Nursing colleagues.

About you

  • Have knowledge and experience of applying statutory legal framework and policies including the Care Act 2014, the Mental Capacity Act 2005 and Making Safeguarding Personal.
  • Be confident in working at pace in challenging situations, to make sound decisions quickly to support those in crisis and to ensure people are seen and supported in a timely manner.
  • Have experience of working with health colleagues, voluntary sector organisations and other partner agencies to help people find solutions to maximise their independence and resilience and enable people to take positive risks and achieve their goals.
  • Demonstrate initiative and be able to respond independently to unexpected situations.
