An exciting opportunity has arisen to join us on our new older adults ward, specialising in the care of older adults experiencing mental health conditions including dementia.
We provide 24-hour care within our purpose-built assessment and therapy unit, working within a multidisciplinary approach that optimises each individual\βs recovery.
We are a vibrant ward with an MDT-focused pathway including non-pharmacological approaches. We are activity-driven and promote person-centred care which supports both our clients and their carers.
The successful candidate will be responsible for further assessment, planning, and implementation of care using an MDT approach.
You will be responsible for the appropriate clinical risk assessment of clients within our care.
You will provide effective advice, guidance, and involvement in decision-making processes alongside our clients, culminating in the development and oversight of care planning with MDT support.
You will oversee admission, assessment, treatment, and discharge for individuals within our care.
You will be responsible for the care and assessment of individuals as a named nurse and for the development of care plans within this role.
