The predicted salary is between 37338 - 44962 £ per year.
This is an exciting time for the Neurodevelopment Service and specifically for the ADHD Post Diagnostic Team in North Lincolnshire which is looking to expand due to additional investment. We are looking for a Nurse Practitioner to join our young person and family centred service.
Responsibilities
- Lead on agreed post diagnostic ADHD interventions for children and young people and work in collaboration with parents/carers, other professionals and agencies to ensure the delivery of agreed plans of support.
- Work as part of the multi-disciplinary team running your own ADHD clinics alongside other practitioners in the team.
- Have access to regular team and individual supervision, and specific designated medical practitioner support.
- Work as an integral member of the Children and Young Peoples Mental Health (CAMHS) multi-disciplinary team, actively promoting effective team working and continuous service improvement.
- Demonstrate experience and knowledge of nurse prescribing.
- Support the implementation of a medication review clinic on the ADHD pathway, working closely with Consultant Psychiatrists, education and family services and ensuring young person and family involvement within the care delivery.
- Be responsible for the assessment, provision and evaluation of interventions and care of children and young people with ADHD, and their families and carers, in all settings relevant to service delivery.
- Attend multi-agency meetings to develop and share plans of care to meet the needs of complex young people within the pathway.
- Support the transition of young people to adult services ensuring continuity of care working in partnership with appropriate adult services.
Requirements
- Experience and knowledge of nurse prescribing.
- Ability to manage your own diary and be creative in delivery of services.
- Commitment and enthusiasm to provide patient- and staff-centered care, with a focus on being a place where people would be happy for their friends and family to be treated.
