Permanent Post based in North Lincolnshire

The Specialist Community Public Health Nurse (SCPHN) will work as part of an integrated team contributing to family health needs assessment. The post holder will lead the 0-5 Healthy Child Programme and work in partnership with families, education, and other agencies to develop, implement, and evaluate packages of care. The health visitor role is a vital part of the 0-19 offer.

Clinical Nursing Practice

  1. Deliver the commissioned service.
  2. Utilise specialist skills and knowledge to assess the health and health-related needs of children and their families. Develop, deliver, and review packages of care to ensure effective treatment and referrals, maximising health outcomes.
  3. Work as an autonomous practitioner, assessing and delivering care in various settings, and assuming professional and legal accountability for all aspects of work in line with trust objectives and guidance.
  4. Ensure adherence to nurse prescribing policies when diagnosing, prescribing, and treating clinical conditions.
  5. Undertake child health promotion, building therapeutic relationships with individuals, families, and communities.
  6. Identify and minimise risks to ensure children are protected and safe, working in partnership with families and agencies.
  7. Complete accurate, timely records on electronic systems in line with NMC and trust guidelines.
  8. Provide evidence-based care, advice, and support to children and families.
