Job overview:

Please note - this post is based in North Lincolnshire. The community nurse will work as part of an integrated team in North Lincolnshire contributing to family health needs assessment. Working with families, children, and young people age 5-19 (25 SEND) years. The post holder will work in partnership with the family and other agencies to develop, implement and evaluate packages of care on an individual basis and with groups. The community nurse will contribute to the wider public health agenda for Children and Young People through individual, group and community programmes of care. This post offers a great opportunity for a Registered Nurse (RN/RSCN/RNC) with post qualification experience. This is an exciting opportunity for the successful candidate to further develop nursing skills within the community setting and deliver quality, evidence-based care.

Main duties of the job:

  • The organisation is supportive of training needs and there is an excellent learning and development programme.
  • You will receive regular safeguarding, management and clinical supervision and will have support from the leadership team.
  • The community staff nurse will work as part of an integrated team contributing to the family health needs assessment when working with families, children and young people age 0-19 years and up to 25 for young people with SEND.
  • The post holder will work in partnership with the family and other agencies to develop, implement and evaluate packages of care on an individual basis and with groups.

  • NMC registration
  • Safeguarding knowledge/experience
  • Evidence of multi-agency working

You must have appropriate UK professional registration. This post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service.
