About the Role
The Adult Community Mental Health Teams within Rotherham, Doncaster and South Humber NHS Foundation Trust have an exciting opportunity for a Band 6, community mental health nurse to join the Central AMH Locality Team. The Central Locality Team provides support to individuals with complex and enduring mental health issues, offering a range of management strategies and interventions including various psychological therapies.
Applicants must demonstrate a flexible approach, evolving around the needs of service users, their families and significant others. The role involves working as a team member with the responsibility of a caseload across traditional boundaries of Adult and the voluntary sector services. The service ethos is focused on recovery and social inclusion and therefore offers successful applicants an exciting and demanding opportunity to improve people’s lives.
We are looking for an enthusiastic person with excellent team working skills, who is keen to develop their knowledge and skills in this area. You should be a competent and motivated practitioner with appropriate demonstrable experience. You will work as part of a multi‑disciplinary team providing assessment and therapeutic intervention in the community.
Key Responsibilities
- Lead and contribute to the provision of a comprehensive range of community services as a key worker for a defined caseload.
- Deliver appropriate and effective evidence‑based care to meet the needs of individuals on a clinical caseload.
- Work collaboratively with patients and their carers to facilitate and promote rehabilitation and recovery, helping patients to achieve and maintain their optimum level of mental health functioning and quality of life in the community.
- Provide assessment and therapeutic intervention in the community as part of a multidisciplinary team.
- Case‑manage across traditional boundaries of Adult and the voluntary sector services.
- Develop a flexible approach, evolving around the needs of service users, families and significant others.
- Support individuals with complex and enduring mental health issues and engage with management strategies and interventions including various psychological therapies.
