The predicted salary is between 36000 - 60000 £ per year.
Due to maternity leave, there is an opportunity to work into the North Lincolnshire Care Home Liaison team for a 12 month fixed term / secondment.
We are looking for an enthusiastic and motivated Band 6 Care Home Liaison Nurse who wants to participate and contribute to achieving excellent standards of quality care within a Care Home environment. The applicant will work alongside the Care Home to provide specialist support, advice, education and support interventions to the Care Home staff.
In return for this commitment, we will support and develop your potential and skills through regular supervision and professional development in this busy and fulfilling role within our friendly Trust.

Main duties of the job

To provide specialist support and advice to referring Care Homes, preventing unnecessary delays.

To promote and assist in the maintenance of the highest attainable standard of nursing care in a Care Home setting, drawing on examples of current best practice.

To deliver education, support interventions, and implement processes with consequent improved patient outcomes, therefore supporting the achievements of Rotherham Doncaster and South Humber NHS Foundation Trust and Rotherham Foundation Trust internal performance indicators.

To undertake clinical effectiveness and audit activities throughout the year.

To promote and assist in effective ward discharge and placement preparation for both the patient and their family / carer.
